- Employee number (unless we assign)
- Home Department Number
- Social Security Number
- Name (first name, middle initial, last name)
- Rate of pay (hourly, daily or salary)
- Marital Status
- Exemptions (Federal and State)
- The State and Local tax to be deducted
- Any additional taxes to be deducted
- Hire Date
- Birth Date
- Address
- Hours worked or salary to be paid during this payroll period
